Travel/Contract - LPN - Long Term Care - Up to $40HR!

Licensed Practical Nurse

This role places a licensed practical nurse in a long-term care and rehabilitation setting for an eight-week assignment in or near Jacksonville, IL. The position offers direct patient care, collaboration with interdisciplinary teams, and competitive pay and benefits.

Key Responsibilities:

• Provide direct nursing care to residents in long-term care and rehabilitation settings
• Collaborate with RN supervisors, physicians, therapy staff, and interdisciplinary team members to support individualized resident care plans
• Maintain accurate, timely documentation of patient assessments, care provided, vital signs, and reporting of changes in condition
• Execute medication administration and treatment assistance in accordance with established protocols, scope of practice, and patient safety guidelines
• Document incident reporting, resident response to care, and follow-up needs to ensure continuity of care across shifts
• Work rotating shift schedule and collaborate with interdisciplinary teams
• Support resident care, documentation, and care plan implementation in a clinical environment

Qualifications:

• Provide proof of completion of an accredited LPN/LVN nursing program
• Maintain a current Basic Life Support (BLS) certification
• Maintain an active CPR/BLS for Healthcare Providers card (as required by facility policy)
• Provide evidence of current certification(s) required by the state/facility for LPN practice (as applicable)
• Provide at least 1 year of LPN experience in Long Term Care and/or Rehabilitation settings
• Demonstrate experience supporting resident care, documentation, and care plan implementation in a clinical environment
• Execute experience working with a rotating shift schedule and collaborating with interdisciplinary teams
• Maintain an active Illinois LPN license (Facility Must Haves)
