Active New York State Physical Therapy license required to start the assignment. BLS certification from AHA or ARC may be required. Minimum 1 year of recent clinical experience in a skilled nursing facility or long-term care setting preferred. Experience with restorative nursing programs, functional mobility training, transfers, and fall risk assessment preferred. Proficiency with electronic medical records and documentation in SNF settings preferred. Assignment Details Facility Type: Skilled Nursing Facility. Shift Name: Days.
