Responsibilities
- Conduct medical necessity determinations and utilization reviews
- Perform discharge planning and coordinate transitions of care
- Manage care coordination, including communication of changes in plan of care
- Connect patients with appropriate community resources to support continued recovery
- Complete timely documentation of initial screenings and discharge plans in the Epic charting system
- Collaborate with interdisciplinary teams and apply knowledge of DRG/MS concepts, documentation, and appeals as needed
- Maintain timely, accurate charting and adherence to HIPAA regulations

Requirements
- Active RN license in Idaho or a compact state at time of submittal
- BLS (Basic Life Support) certification
- Epic charting experience required
- 2+ years of relevant RN experience preferred; previous charge experience and community hospital experience preferred
- Ability to work in an acute hospital/inpatient setting
- Experience with medical necessity determination, utilization review, and transitions of care is preferred
- Strong documentation, communication, and care coordination skills
