\n \nPosition Overview
\nAs a Registered Nurse (RN) , you will provide skilled nursing care to patients in their homes, working under physician orders and care plans, while collaborating with a dedicated clinical team.

✅ Key Responsibilities
\n- \n
- Deliver skilled nursing care based on individualized care plans \n
- Conduct initial patient assessments and ongoing evaluations \n
- Coordinate care and communicate changes with physicians and team members \n
- Educate patients and families on care and health management \n
- Maintain accurate, timely clinical documentation (including OASIS) \n
- Participate in care planning, case conferences, and quality improvement efforts \n
- Supervise LPNs, CNAs, and HHAs per regulatory guidelines \n
- Support patients’ rehabilitative and preventive care needs \n
Qualifications
\n- \n
- Active RN license \n
- Graduate of an accredited nursing program \n
- At least 1 year of nursing experience (preferred) \n
- Strong clinical judgment, communication, and organizational skills \n
- Current CPR certification \n
- Ability to work independently and manage time effectively \n
