RN Case Manager

Job Summary:

We seek a highly skilled and experienced Registered Nurse with at least 2 years of acute care experience to serve as a Case Manager. The role involves providing skilled nursing care, patient assessments, medication administration, treatment planning, and collaborating with interdisciplinary teams. The nurse will support patients in various care settings, ensuring effective communication and coordination of care. This position requires flexibility to float between units as needed and includes weekend work every other week. Responsibilities: Required Skills & Certifications:

• Active Registered Nurse license in the state of practice.
• Minimum of 2 years acute care nursing experience.
• Proficiency in clinical assessment, medication administration, and treatment planning.
• Strong interdisciplinary team collaboration skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.

Preferred Skills & Certifications:

• Experience with case management in acute care settings.
• Familiarity with Epic electronic health record system.
• Bachelor's degree in Nursing or related field.
• Driver's license, car insurance, and reliable automobile for travel if required.

Special Considerations:

• Required to work every other weekend.
• Call is not required for this position.
• Floating between like units based on patient care needs.
• Facility assists with housing, including realtor support if needed.

Scheduling:

• Work schedule includes every other weekend shifts.
• Shift assignments and float requirements determined by unit needs.
