NURSE (RN/LPN), MENTAL HEALTH, ACT TEAM - NELSON (FULL TIME)

Responsibilities Coordinate care with community healthcare providers and our team psychiatric provider Participate in team meetings, case conferences, and ongoing professional development Administer injections and lab orders Facilitate health promotion and illness prevention activities, including lifestyle management and symptom management. Engage with consumers and their families to support treatment adherence and improve health outcomes. Maintain accurate and up-to-date consumer records and documentation. Provide crisis intervention and support to consumers experiencing acute psychiatric episodes. Qualifications Position requires strong organizational and time management skills, excellent writing skills, flexibility, and self-motivation. Valid VA driver's license and acceptable driving record is required. The incumbent in this position must be a registered nurse (RN) with one year of experience in the provision of mental health services to adults with serious mental illness or a licensed practical nurse (LPN) with three years experience in the provision of mental health services to adults with serious mental illness.
