Job Description

RESIDENCY: Candidates must have been legal residents of the following jurisdiction(s) for at least four months immediately preceding the date of the application: Livingston County, or one of the following Counties: Allegany, Genesee, Monroe, Ontario, Steuben or Wyoming.

APPLICANT QUALIFICATIONS: In order for an application to be approved, the applicant must clearly show the training, education, experience, and/or licensure/certification stated in the Typical Qualifications. All qualifying information must appear on the application form. No credit will be given for a Work Experience description that is unclear.

The work involves responsibility for overseeing the Assisted Outpatient Treatment (AOT) and After Hours Mobile Crisis programs. The Assisted Outpatient Treatment Coordinator manages the daily operations of the program by linking the legal system, clinical team, and community resources to ensure high-risk individuals adhere to court-ordered treatment. Work is performed in the office and in the field. Employee must be willing and accessible to work evenings and weekends as needed. Supervision is exercised over Mental Health Crisis Therapists. The work is performed under the general supervision of the Deputy Director of Community Mental Health Services, with leeway allowed for the exercise of independent judgment within the limits of departmental policy. An Assisted Outpatient Treatment Coordinator does related work as assigned.

Examples of Duties

• Administers and oversees local implementation of Assisted Outpatient Treatment (AOT) services and complex care needs;
• Receives, reviews, and investigates referrals for potential AOT candidates;
• Provides clinical supervision and direction to a professional mental health staff;
• Coordinates clinical assessments and eligibility determinations;
• Facilitates development and filing of AOT petitions;
• Facilitates case review meetings for at risk and high-risk individuals;
• Develops safe crisis response, discharge planning, and treatment plans;
• Develops policies and procedures specific to AOT and After Hours Mobile Crisis programs;
• Collaborates with legal counsel, hospitals, courts, and treatment providers throughout the AOT process;
• Reviews and monitor plans submitted by hospitals and providers in consultation with AOT psychiatrists;
• Monitors adherence and compliance to court-ordered treatment plans and service delivery;
• Coordinates access to community-based services, including care management, housing, substance use treatment, and outpatient mental health services;
• Facilitates multi-disciplinary case management review meetings to ensure legal, clinical, and procedural requirements are met;
• Tracks outcomes, maintains required documentation, and completes Office of Mental Health (OMH) reporting requirements;
• Advises professional mental health staff regarding safety plans for high-risk individuals in crisis;
• Participates in statewide training, quality improvement initiatives, and OMH oversight activities;
• Develops local procedures and community partnerships to ensure timely access for individuals under AOT orders;
• Provides AOT training to community partners as needed; and
• Performs other related duties as assigned.

Typical Qualifications

MINIMUM QUALIFICATIONS: Applicants must meet all requirements described in the following table in order to be approved for examination and/or appointment:

Training/Education - Master's Degree in a behavioral health field.

General experience - Two years of full-time, paid professional experience providing mental health services or mental health case management services to patients.

Supervisory experience - N.A.

License/Certification - Licensed and currently registered by the New York State Education Department as a Master Social Worker, Clinical Social Worker, Mental Health Counselor, Marriage and Family Therapist, Licensed Creative Arts Therapist, or Licensed Psychologist; AND possession of a valid New York State driver's license.

Supplemental Information

1. Appropriate professional licensure must be maintained throughout employment.
2. Appropriate driving license must be maintained throughout employment.
3. A driving record evidencing safe and responsible vehicle operation is required.
4. Must authorize and execute any necessary releases to enable the appointing authority to verify licensure and driving history involving violations and/or crimes.

PROVISIONAL APPOINTMENT NOTICE: An individual who receives a provisional appointment must take the examination for the position title on the next occasion it is offered. In order to be eligible for a permanent appointment to the position, the provisional appointee must pass the exam and be ranked 1, 2, 3, or be tied in score with the individual ranked number 3 of those who are willing to accept appointment to the position. It should be understood that even if the provisional appointee is eligible for appointment, the employer may choose another candidate to fill the position on a permanent basis. Questions regarding provisional appointments should be directed to the Livingston County Human Resources Department.

NOTE: Livingston County residency is not a requirement for this position. However, Livingston County residents will be given preference in the hiring process.
