POSITION OVERVIEW:

This Aging Care Management Supervisor I position for the Area Agency on Aging of Somerset County is a supervisory position within the care management unit. An employee in this class title supervises a sub-unit of Aging Care Managers who provides services to elderly consumers in an assigned area within Somerset County. This individual may supervise the provision of direct services in any one of several programs within a moderately sized Area Agency on Aging. An assignment including a combination of directly provided services and contracted services is appropriate. Supervision is received from a higher-level supervisor or a program administrator through an evaluation of written reports and day to day conferences and discussions.

EDUCATION/EXPERIENCE:

Any combination equivalent to education and experience that provides the required skill and knowledge is qualifying. Typical qualifications would be equivalent to:

Three years of experience in public or private social work and a Bachelor’s Degree or

Any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.

EMPLOYMENT STANDARDS:

To perform this job successfully, an individual must possess the knowledge of social casework principles and methods, knowledge of current social, economic, and health problems and community resources as related to older adults, knowledge of individual and group behavior and ways of working effectively with older adults who have social, economic, emotional, and/or health problems and knowledge of basic principles and methods of program interpretation and community organizations. An employee in this class title must have the ability to work effectively with older adults and aid them to grow in constructive use of their potential in adjusting to their specific problems, the ability to evaluate work of subordinates and to provide technical assistance and guidance, the ability to orient and instruct subordinates on procedures, techniques, laws and regulations, the ability to establish and maintain effective working relationships with consumers, other members of the staff, outside agencies and institutions, and the general public, the ability to clearly express ideas orally and in writing, the ability to read, send and format e-mail, the ability to monitor/manage data software, and possess skills  in the development of plans to address the needs of older adults.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

An employee in this classification is responsible for performing assignments supervisory in nature and is carried out in accordance with established federal, state and local regulations and policies. Specific job duties include the following:

·         Supervises a care management unit including Aging Care Managers in all required duties in an Area Agency on Aging.

·         Ensures that all rules and regulations are met within the statewide reporting program known as the Social Management Assessment System

·         Trains and supervises Aging Care Managers in the rules, regulations and procedures of aging programs.

·         Reviews and analyzes case records for completeness and adherence to established standards.

·         Evaluates staff performances and makes recommendations regarding personnel actions.

·         Completes case assignments and case reviews in all direct service program areas.

·         Prepares reports and correspondence, and maintains necessary records.

·         Completes required reports for internal operations, direct social service programs and any other required agency information.

·         Provides in house training through utilization of Pennsylvania Department of Aging material and local supplements.

·         Completes comprehensive assessments for direct service programs, as needed.

·         Complete case reviews and care management support in the state reporting system Statewide Data Reporting System.

·         Support direct care manager in training needs and assist in Statewide Data Reporting System.

·         Assist Administration in the validation of data through the utilization of reports/rosters within the Statewide Data Reporting System.

·         Oversees the Protective Service Program while participating in on-call rotation

·         Attends work on a regular basis.

·         Maintains a professional appearance and demeanor.

·         Attends all required meetings and trainings.

·         Adheres to and promotes maintenance of Agency confidentiality guidelines.

·         Adheres to and promotes Agency policies and procedures.

·         Operates and maintains Agency vehicles in a safe manner.

·         Promotes safety awareness.

·         Performs other related duties as required.

While performing the duties of this job, the employee is regularly required to sit for extended periods, use a computer keyboard, write, talk and hear; stand and lift up to ten (10) pounds occasionally.  The noise level in the work environment is usually moderate.
