This opportunity is for a pharmacist focusing on Transitions of Care and Ambulatory Care within a hospital setting. The role centers on working closely with patients prior to discharge to provide medication education, counseling, and assistance with medication refills before they leave the hospital. This “meds to beds” approach helps ensure patients adhere to their prescribed medication regimens after discharge, promoting better health outcomes.

Key Responsibilities:

• Educate and counsel patients on their medications before hospital discharge
• Assist with medication refills to support continuous treatment after leaving the hospital
• Collaborate with healthcare teams to improve medication management during care transitions
• Support ambulatory care pharmacy services as needed

Qualifications / Desired Experience:

• Licensed pharmacist with hospital experience preferred
• New PGY1 residents highly encouraged to apply; non-PGY1 candidates considered
• Background in retail or outpatient pharmacy is acceptable
• Fluency in Spanish is a strong plus, enhancing patient communication
• Ability to work flexible 8-hour shifts, primarily 1st and 2nd shifts, with occasional 3rd shifts required

Location:

• Position is based in Van Nuys, CA, at a major county hospital within a large health system that encompasses multiple outpatient clinics and hospitals.

Schedule & Hours:

• Flexible 8-hour shifts (mainly day and evening), with occasional night shift coverage

This role is offered as a contract position with a potential path to permanent employment. This arrangement helps address immediate operational needs while providing a transition into a long-term career within the health system.
