Schedule Expectations
Full-Time: 25+ visits per week
Part-Time: 6+ visits per week
PRN: 1 shift every 6 months
*Open roles: PT, OT & SLP
Earn up to $93,800 per year as Physical Therapist (PT) || What You'll Do:
- Conduct PT evaluations and reassessments.
- Develop/update individualized treatment plans based on patient needs and goals.
- Provide skilled therapy to improve mobility, strength, balance, coordination, and function.
- Establish and monitor home exercise programs.
- Educate patients/families/caregivers on goals, progress, safety, fall prevention, and assistive-device use.
- Monitor progress and adjust treatment plans as clinically appropriate.
- Supervise and communicate with PT Assistants when applicable.
- Complete timely, accurate EMR clinical documentation.
- Coordinate visits with patients, caregivers, and agency staff.
- Collaborate with physicians and interdisciplinary teams to ensure quality, continuity of care.
- Participate in care conferences and discharge planning as needed.
- Follow infection control, patient safety, confidentiality, and agency policies.
- Travel to patients’ homes within the assigned 20-mile service area.
PT - Qualifications
- Graduate degree in PT from a CAPTE-accredited program.
- Current, unrestricted Texas PT license.
- Pediatric/home-health experience preferred, not required.
- Current CPR/BLS certification.
- Current TB screening (within 1 year) or chest X-ray (within 5 years), as applicable.
- Valid driver’s license and acceptable driving record.
- Reliable transportation and current auto liability insurance.
- Willingness to travel locally within ~20 miles of home.
- Flexible schedule based on patient needs.
- Strong clinical, communication, patient-education, and time-management skills.
- Ability to work independently in homes and collaborate with interdisciplinary teams.
- EMR proficiency and timely clinical documentation preferred.
- Ability to meet background-check and healthcare employment requirements.
- Bilingual English/Spanish preferred, not required.

Earn up to $92,300 per year as Occupational Therapist (OT) || What You'll Do:
- Conduct OT evaluations and reassessments.
- Develop/update individualized treatment plans based on patient needs and goals.
- Provide skilled OT to improve fine motor skills, coordination, sensory processing, strength, self-care, and daily living skills.
- Monitor progress and adjust care plans as clinically appropriate.
- Develop home exercise and therapeutic activity programs.
- Educate patients/families/caregivers on goals, progress, safety, adaptive techniques, and home activities.
- Recommend adaptive equipment/assistive devices and provide training as needed.
- Complete timely, accurate EMR documentation, including evaluations, progress notes, recertifications, and discharge summaries.
- Supervise and communicate with OT Assistants when assigned.
- Coordinate visits with patients, caregivers, and agency staff.
- Collaborate with physicians and interdisciplinary teams to ensure quality, continuity of care.
- Participate in care coordination, conferences, and discharge planning as needed.
- Travel to patients’ homes within the assigned 20-mile service area.
OT - Qualifications
- Graduate of an accredited OT program.
- Current, unrestricted Texas OT license.

Earn up to $86,000 per year as Speech Language Pathologist (SLP) || What You'll Do
- Perform comprehensive speech, language, communication, and feeding evaluations and reassessments.
- Develop and update individualized treatment plans based on each patient’s clinical needs and goals.
- Provide skilled interventions to improve speech, language, communication, cognitive-communication, and feeding skills, when appropriate.
- Establish home programs and educate patients, families, and caregivers on treatment strategies, goals, progress, and safety.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete timely and accurate evaluations, progress notes, recertifications, discharge summaries, and other required EMR documentation.
- Supervise and communicate regularly with Speech Therapy Assistants, when applicable.
- Provide and document all required direct and indirect STA supervision in accordance with Texas requirements and RHC policy.
- Coordinate visit schedules with patients, caregivers, assistants, and agency staff.
- Collaborate with physicians and interdisciplinary team members to support continuity and quality of care.
- Participate in care coordination, care conferences, and discharge planning as needed.
- Travel to patients’ homes throughout the assigned 20 mile service area.
SLP - Qualifications
- Master’s degree or higher in Speech-Language Pathology, Communication Sciences and Disorders, or a related qualifying field.
- Current, unrestricted Texas Speech-Language Pathologist license.
- Pediatric or home-health experience preferred but not required.
- Current CPR/BLS certification.
- Current TB screening within the past year or chest X-ray within the past five years, as applicable.
- Valid driver’s license with an acceptable driving record.
- Reliable transportation and current automobile liability insurance.
- Ability and willingness to travel locally within approximately a 20-mile radius of home.
- Ability to maintain a flexible schedule based on patient needs.
- Strong clinical, communication, patient-education, and time-management skills.
- Ability to work independently in patients’ homes and collaborate with an interdisciplinary care team.
- Proficiency with electronic medical records and timely clinical documentation preferred.
- Ability to satisfy applicable background-check and healthcare employment requirements.
- Must have at least two years of professional experience providing direct patient services and meet Texas requirements to supervise Speech-Language Pathology Assistants, if assigned supervisory responsibilities.
- Bilingual English/Spanish skills preferred but not required.
