Home Care Speech Language Pathologist, Chenango County Region

Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluation and therapy in patients' homes, enhancing communication, cognition, and swallowing abilities. Responsibilities Conduct assessments, develop and adjust individualized treatment plans, and document progress in compliance with regulations. Educate patients and caregivers on communication strategies and safe swallowing techniques. Collaborate with interdisciplinary teams, participate in care planning, and recommend assistive technology when needed. Maintain licensure, stay current on best practices, and contribute to team development. Qualifications Qualifications include a Master’s degree, current NY licensure, CCC from ASHA or CFY status, and CPR certification. Experience with dysphagia and neurogenic disorders is preferred.
