Nurse Practitioner / Physician Assistant - Primary Care

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team‑based care to patients residing at Assisted Living and Memory Care communities in and around Forest Lake, MN. Position Overview Provide on‑site, team‑oriented primary care to patients within Assisted Living and Memory Care communities Full‑time providers care for patients Monday‑Thursday; option for Fridays to be a clinical or an administrative day Manage a panel of 200 patients (approximately 12‑16 patients per day with 14 being the average) Design and manage your schedule Develop and implement care plans appropriate to patients and their desires Receive clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers Access to diagnostics such as labs and x‑rays as needed Collaborate with community partners within home care, hospice, and other resources as appropriate Qualifications Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program Board Certified/Board Prepared through AANP, ANCC, or NCCPA Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice Ability to obtain and/or maintain DEA certification for the state of required work Proven ability to communicate effectively with strong verbal skills Excellent interpersonal and customer service skills Must have a valid and unrestricted driver’s license and minimum state‑required liability auto insurance; local travel is required Preferred Qualifications 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant Benefits Compensation Base + Quality/Productivity: $125k - 170k Generous and ongoing CME Offering Unlimited PTO and eight observed holidays Health Insurance, Dental and Vision Insurance Company‑paid Life Insurance Company‑paid Short‑ and Long‑term Disability Health Savings Account (HSA) & Flexible Spending Account (FSA) Retirement plan with 4% matching contributions Pay Transparency $125,000—$170,000 USD #J-18808-Ljbffr
