Job Description
We are seeking a licensed Physical Therapist (PT) to provide in-home physical therapy services to patients in the community. This is a great opportunity for a therapist looking for flexible scheduling , consistent referrals, and a supportive agency team.
Responsibilities:
Provide skilled physical therapy services in the home setting
Complete patient evaluations and develop individualized treatment plans
Deliver therapeutic exercises, gait training, balance training, and mobility support
Educate patients and families on home exercise programs and safety
Complete accurate and timely documentation in compliance with agency standards
Communicate progress and updates with the clinical team as needed
Qualifications:
Active New Jersey Physical Therapist license (required)
Strong communication and interpersonal skills
Reliable transportation
Ability to work independently and professionally
