Role Description The Hospice On-call Registered Nurse role at Heritage Hospice is a full-time, hybrid position based in Evansville, IN, with some work-from-home flexibility as appropriate for administrative and documentation tasks. Day-to-day responsibilities include providing direct patient care, performing assessments, developing and implementing nursing care plans, administering medications and treatments, and monitoring patient responses. The nurse collaborates with physicians and interdisciplinary team members, communicates clearly with patients and families, and supports education on health conditions and treatment plans. The role also involves accurate documentation in electronic health records, adherence to clinical protocols and regulatory requirements, participation in quality improvement initiatives, and contributing to a safe, respectful care environment. The Registered Nurse may coordinate care transitions, support telehealth or remote follow-up, and assist with case management activities as needed. Qualifications Active Registered Nurse (RN) license in the state of Indiana (or ability to obtain), in good standing Associate or Bachelor’s degree in Nursing from an accredited program; BSN preferred Demonstrated clinical skills in patient assessment, care planning, medication administration, and monitoring Strong communication and interpersonal abilities for working with patients, families, and interdisciplinary teams Proficiency with electronic health record (EHR) systems and basic computer skills for documentation and remote work Knowledge of infection control, safety standards, and regulatory and compliance requirements in healthcare settings Ability to prioritize, manage multiple patients and tasks, and maintain accurate, timely documentation Commitment to patient-centered, culturally sensitive care and maintaining professional boundaries Experience in acute care, long-term care, or community health nursing is beneficial #J-18808-Ljbffr HOLIDAY HEALTHCARE, LLC
