We’re hiring a Patient Care Coordinator – a dedicated and organized professional who ensures seamless coordination of patient care, supports clinical teams, and serves as a vital link between patients, families, and hospice leadership. This role is essential to maintaining quality care and compliance while fostering compassionate communication. What You’ll Be Doing Maintain clinical responsibility for your team and act as liaison between the Director of Patient Care Services (DPCS), clinical associates, patients, families, and the community Manage clinical associates’ schedules, including holidays, vacations, weekends, and daily assignments Investigate issues, notify DPCS for corrective action, and review hospice documentation for accuracy Participate in implementing clinical policies and procedures aligned with Silverado’s mission and goals Ensure compliance with local, state, and federal regulations and hospice standards Assist in planning and implementing hospice Interdisciplinary Group (IDG) conferences Qualifications Previous healthcare experience in an administrative setting required; hospice experience preferred Strong background in home health, healthcare, or hospice preferred Minimum of 1 year experience in a clinical healthcare setting (hospital, clinic, or home health agency) Reliable transportation to meet attendance and punctuality standards (driver’s license not required unless specified) Fortune Magazine Best Workplace in Aging Services™ Certified Great Place to Work® Paid training and strong internal mobility Competitive commission and bonus plans Schedule: Monday - Friday, 8:00am - 5:00pm #LI-AH1 Anticipated pay range $29 - $31 USD Silverado offers the following benefits to eligible employees: 401(k) matching Health, Dental and Vision Insurance Flexible Spending Account (FSA) Paid time off Tuition reimbursement and professional development assistance Referral program Other amazing benefits such as Pet Insurance, Auto and Home Insurance and Discount Programs! * Silverado does not accept candidates from outside recruiters or agencies for this role. * California Employees: For roles requiring driving, the driving requirement has been deemed essential to the performance of this role due to specific business necessity.
