RN Case Manager - Hospice

Location: Lebanon, NH and White River Junction, VT areas

Assignment Duration: This is a 13-week contract position.

Schedule: Full Time, Days

Job Description: As an RN Case Manager in Hospice, you will provide vital support and care to patients with terminal or chronic illnesses. Your role involves coordinating care, developing individualized care plans, and ensuring comfort and pain management for patients and their families.

Responsibilities:

• Develop care plans in collaboration with physicians, patients, and caregivers upon admission.
• Periodically review and recertify patient care plans as necessary.
• Coordinate care through interdisciplinary communication and case conferences.
• Provide ongoing assessments of patients' clinical, functional, and emotional statuses.
• Communicate significant changes to the patient's physician and care team.
• Document evaluations and progress towards goals.
• Teach care techniques and end-of-life symptom management to patients and caregivers.

Required Experience / Certifications / Licensure:

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license.
• Graduate of an accredited school of nursing.
• Preferred experience in Medical/Surgical/Oncology nursing or hospice care.
