VNA is looking for an RN Case Manager.Job Summary: The RN Case Manager works collaboratively with physicians and the interdisciplinary care team to assess, plan, coordinate, and deliver skilled home health care to patients and their families. This role focuses on providing supportive, patient-centered care that promotes comfort, safety, and quality of life. The nurse utilizes the nursing process to manage patient care, coordinates services with internal and external providers, and ensures compliance with payer guidelines. The RN Case Manager also prepares patients and caregivers for discharge when appropriate and connects them with community resources to support continuity of care.Status: Full-TimeShift: Monday - Friday w/ rotating weekendsEducation/Experience/Skills/Qualifications:Active Registered Nurse license in CaliforniaMinimum of one year nursing experience requiredHome Health experience requiredTwo years of recent medical-surgical nursing experience preferredThis position is based in Monterey, CA and requires daily travel throughout the Monterey Peninsula and surrounding coastal communities. The salary range provided reflects the lowest to highest amount we reasonably anticipate paying for this position at the time of posting. Compensation is influenced by labor market conditions and various factors, including but not limited to: prevailing minimum wage in the area, relevant education, experience, skills, geographic location, performance, travel requirements, sales or revenue targets, and business needs. It is not typical for a new hire to be offered the highest end of the salary range.
