Duties and Responsibilities

• Coordinate and process referrals
• Coordinate and process hospital discharges
• Submit and track authorizations
• Interface with corporate management, administrative personnel, medical, dental and behavioral staff
• Identifies the health needs of the patient and the expectations of the family.
• Makes an assessment of the resources and support network at the patient’s disposal.
• Assists in development, implementation, and revision of individual treatment plans; assures that services provided are specified in the Treatment Plan; and monitors progress toward treatment goals.
• Maintains an active caseload, providing interventions as needed and within area of expertise and limits of credentials; assures procurement of additional services as needed.
• Reviews and facilitates, if appropriate, mental health referrals from other sources such as primary care, school health, etc. Offers alternative recommendations where necessary.
• Maintains patient and program records in accordance with applicable standards and regulations, grant requirements, etc.
• Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.
• Participates in continuing education activities, remaining knowledgeable in area(s) of expertise.
• Attend meetings, as appropriate, and meet regularly with supervisor to exchange pertinent information and receive supervision.
• Participates in all safety programs, which may include assignment to an emergency response team.
• Other responsibilities as assigned to create a smoothly run efficient Clinic.

 

Education

• High School Diploma or equivalent (required).
• Associate's degree in health administration, Medical Office Administration, ora related field (preferred).
• Certification in medical office administration ora relatedcertification (e.g., Certified Medical Assistant, Referral Coordinator certification) is a plus.
• Minimum of 1-2 years of experience in a healthcare setting, preferably ina referral coordination, medical office, or patient services role.
• Experience with insurance verification and authorization processes is highly desirable.
• Familiarity with electronic health record (EHR) systems and scheduling software.
• Knowledge of medical terminology, patient referral processes, and healthcare regulations, including HIPAA compliance.
• Experience working in a Federally Qualified Health Center (FQHC) or community health setting is a plus.
