$1749 - $3413/wk Provider compensation offers are based on skill level, experience, and qualifications. Compensation may also be impacted by the critical nature of the need and vary by assignment. In addition to taxable hourly wage, providers may qualify to receive non-taxable daily lodging, meals, and incidental per diems not to exceed published General Services Administration (GSA) guidelines. • Mon Fri, 8 am 5 pm rotating weekends required
• Guaranteed 40 hours per week
• 6.5 visits per day
• Home health physical therapy setting
• EMR: Home Care Home Base (HCHB)
• SOC, recertifications, ROC, evaluations, and daily visits
• Common diagnoses CHF, orthopedic conditions, pneumonia
• 13-week assignment
