LMSW Social Worker

Job Summary The Licensed Masters of Social Worker (LMSW) provides specialized social services support in acute care and medical/surgical settings. This role involves assessing patient needs, coordinating care plans, and collaborating with interdisciplinary teams to ensure comprehensive patient support. The LMSW is expected to have at least two years of relevant experience and demonstrate strong clinical and communication skills. Flexibility to work weekends and holidays as required is essential. Responsibilities Conduct psychosocial assessments and develop individualized care plans for patients in acute care/medical-surgical environments. Coordinate with healthcare providers, patients, and families to facilitate discharge planning and access to community resources. Provide crisis intervention, counseling, and support to patients and families facing complex medical and social challenges. Document assessments, interventions, and patient progress accurately in compliance with regulatory standards. Collaborate with interdisciplinary teams to promote holistic patient care and optimize outcomes. Participate in coverage for weekends and holiday shifts as required. Required Skills & Certifications Active Licensed Masters of Social Work (LMSW) credential. Minimum of 2 years' experience in acute care or medical/surgical social work. Experience working in hospital or healthcare settings. Strong assessment, case management, and crisis intervention skills. Excellent communication and interdisciplinary collaboration abilities. Ability to work independently and adapt to a fast-paced environment. Must meet travel position eligibility requirements including a minimum 3-month separation from previous Providence-affiliated employment. Preferred Skills & Certifications Experience with discharge planning and community resource coordination. Prior experience in a travel social work role. Knowledge of healthcare regulations and compliance standards. Special Considerations Must be willing to cover some weekend shifts. Holiday work is required for one of three major holidays: Thanksgiving, Christmas, or New Year's. Active compact license recognized by Swedish facilities is honored. Travel position requires a minimum 3-month separation from any prior Providence-affiliated employment. Scheduling Standard schedule is five 8-hour shifts, primarily Monday through Friday.
