SafeCare Team Lead

Status: Full-time, 40 hours per week; Non-Exempt

Location: The Matthews House, Fort Collins, CO; hybrid option up to 20% of total work hours

Pay Range: $25.50-$26.50/hour

JOB DESCRIPTION:

The SafeCare Team Lead is a hands-on leadership role responsible for supporting high-quality SafeCare services while supervising two SafeCare staff members and maintaining a small caseload of approximately 4–6 families at a time. This position coordinates and distributes referrals, supports staff with engagement and case planning, and helps ensure families receive timely, relationship-centered services. Strong communication, follow-through, family engagement, and accurate data entry are essential to this role. The Team Lead also delivers the SafeCare model directly with families and supports fidelity, documentation, quality assurance, and collaboration with referral partners. QUALIFICATIONS:

• Associate’s degree in Criminal Justice, Social Work, or a related field is required; a bachelor’s degree in Psychology, Social Work, Child Development, Criminal Justice, Sociology, Counseling, or a related field is preferred.

• 2+ years of experience in family services, early childhood, child welfare, home visiting, prevention, or a related human services field is preferred; prior supervisory or team-lead experience is strongly preferred.

• Knowledge of the SafeCare model or a similar evidence-based parenting or prevention program is preferred.

• First Aid/CPR and other required program certifications are required or must be obtained within agency/program timelines.

• Strong verbal and written communication skills, including the ability to communicate clearly with staff, families, referral partners, and community providers.

• Demonstrated ability to engage families, build trusting relationships, address barriers to participation, and support retention in services.

• Strong organizational skills and ability to manage referrals, staff responsibilities, a small caseload, deadlines, and competing priorities.

• Ability to complete timely and accurate data entry, case notes, assessments, and progress tracking in Community Care Link (CCL), Salesforce, and other required systems.

• Reliable transportation and a valid driver’s license are required; regular travel for home visits, partner meetings, and community-based services is expected.

ESSENTIAL DUTIES AND MAJOR RESPONSIBILITIES

Team Leadership, Supervision & Referral Coordination – 35%

• Provide regular supervision, coaching, support, and accountability for two SafeCare staff members.

• Receive, review, assign, and track SafeCare referrals to support timely outreach and equitable caseload distribution.

• Monitor staff caseloads, family engagement, documentation, and follow-up; help identify barriers and develop solutions when families are difficult to reach or at risk of disengaging.

• Support staff in navigating complex family situations, maintaining professional boundaries, and coordinating effectively with referral partners.

• Promote clear, consistent communication across the SafeCare team and elevate program or family concerns to leadership as appropriate.

SafeCare Service Delivery & Family Engagement – 30%

• Maintain a small caseload of approximately 4–6 families at a time and deliver SafeCare services through home visits and community-based sessions.

• Deliver SafeCare curriculum modules, including parent-child interaction, home safety, and child health, with fidelity to the model.

• Conduct intakes, assessments, goal setting, skill modeling, coaching, and ongoing follow-up with caregivers.

• Build trusting, strengths-based relationships with families and use trauma-informed and culturally responsive approaches to support participation and retention.

• Make warm handoffs and connect families to community resources such as housing, food, childcare, healthcare, behavioral health, and other supports.

Documentation, Data & Quality Assurance – 20%

• Ensure timely and accurate case notes, assessments, service documentation, and progress tracking in Community Care Link (CCL), Salesforce, and SafeCare fidelity tools.

• Review team documentation and data for completeness, accuracy, confidentiality, and compliance with program expectations.

• Support data collection, reporting, quality assurance, and continuous quality improvement requirements for funders, program oversight, and SafeCare fidelity.

• Track referral status, engagement, caseload capacity, and program outcomes and communicate trends or concerns to leadership.

Community Partnership & Program Support – 15%

• Maintain strong communication with DHS, healthcare providers, early childhood partners, behavioral health providers, schools, and other referral partners.

• Coordinate services and communicate appropriately with referral sources to support families and strengthen program engagement.

• Support outreach, community presentations, and partner education to increase awareness of SafeCare and appropriate referrals.

• Participate in training, team meetings, program development, and continuous quality improvement activities.

Assist with other duties as assigned.

SCHEDULE:

• Full-time, 40 hours per week, generally Monday through Friday; hours may vary based on family and program needs.

• Some evenings may be required to accommodate home visits, families, trainings, or community meetings.

• This role is based at The Matthews House in Fort Collins and includes significant community-based work and travel to family homes and partner locations.
