We are dedicated to providing the best resident centered care.
Job Summary: We are seeking a skilled and motivated Occupational Therapist (OT) to join our dynamic rehab and home health team. In this role, you will assess, plan, and implement individualized therapy programs for both short-term rehab patients and home health clients. The ideal candidate is a caring professional who enjoys working in a team environment and is committed to helping individuals regain or maintain their independence in daily living.
Key Responsibilities:
- Evaluate patients' functional abilities and develop individualized treatment plans
- Provide hands-on therapy to improve residents' and patients' ability to perform daily activities (ADLs)
- Document progress and adjust treatment plans as needed
- Educate patients, residents, and caregivers on exercises and strategies to improve function
- Collaborate with interdisciplinary team members, including physical therapists, speech therapists, nursing staff, and social workers
- Ensure compliance with regulatory requirements and documentation standards
- Maintain accurate records using electronic medical record (EMR) systems
Qualifications:
- Bachelor's or Master's degree in Occupational Therapy from an accredited program
- Current state licensure as an Occupational Therapist (OTR/L)
- Excellent communication, organization, and time management skills
- Compassionate, patient-centered mindset with a passion for helping others
