Black Hills Special Services Cooperative is seeking an LPN–Case Nurse to support individuals with developmental disabilities by coordinating their medical care and helping them achieve personal health goals and needs. You'll manage caseloads, monitor medications and treatments, and collaborate with direct support staff and healthcare providers to ensure timely, person-centered care. This position is ideal for a licensed LPN who is organized, communicative, and passionate about making a difference in the lives of others through compassionate nursing and thoughtful case management.Black Hills Special Services Cooperative (BHSSC) is a public entity that is an educational cooperative under the State of South Dakota. BHSSC is comprised of five divisions dedicated to aiding school districts, individuals, and the communities. Wage- $27-$31 per hour based on experienceJob Type-Full-Time, Non-exemptWork Location-Sturgis, SDJob Description-Maintain a caseload of individuals receiving services and support their ongoing health, wellness, and medical needs. Coordinate medical appointments, referrals, preventive care, follow-up services, and healthcare recommendations.Monitor health conditions and communicate changes in health status to ensure timely intervention and support.Assist with medication management, including monitoring medication systems, refills, documentation, and medication-related concerns.Complete health assessments, healthcare reports, care documentation, and nursing-related records in accordance with regulatory requirements.Collaborate with individuals served, families, guardians, healthcare providers, pharmacists, and support teams to ensure coordinated care.Provide health-related training, coaching, and support to Direct Support Professionals and other team members.Promote person-centered healthcare, healthy lifestyles, independence, and informed healthcare decision-making.
