Collaborates with multidisciplinary care providers to monitor, assess and intervene as needed to ensure safe, appropriate and cost-effective medication use.Minimum Requirements:RI Pharmacy license. Pharmacists that provide staffing for inpatient and ambulatory area coverage, and are not solely focused on Medication Reconciliation duties, are required to have active BLS certification as part of their job duties.Pay starting at: $53.45/hour
