Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments and create customized treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate closely with a cross-disciplinary team including nurses, doctors, case managers, and rehabilitation specialists to improve outcomes
• Instruct patients and their families on therapeutic objectives, discharge processes, and at-home exercise routines
• Accurately document evaluations, treatments, and patient progress in compliance with the facility's and regulatory guidelines
• Engage in care team rounds and interdisciplinary meetings to ensure cohesive patient care

Education

• A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Current state licensure as a PT / Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., neurological or geriatric) are advantageous, though not mandatory

Skills

• Prior experience is highly preferred
