- Conduct thorough evaluations of patients in both inpatient and outpatient environments and create customized treatment plans
- Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
- Collaborate closely with a cross-disciplinary team including nurses, doctors, case managers, and rehabilitation specialists to improve outcomes
- Instruct patients and their families on therapeutic objectives, discharge processes, and at-home exercise routines
- Accurately document evaluations, treatments, and patient progress in compliance with the facility's and regulatory guidelines
- Engage in care team rounds and interdisciplinary meetings to ensure cohesive patient care
- A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- Current state licensure as a PT / Physical Therapist
- CPR certification preferred
- Specialized certifications (e.g., neurological or geriatric) are advantageous, though not mandatory
- Prior experience is highly preferred
- Proficiency in treating patients with intricate medical and mobility challenges
- Excellent communication and teamwork abilities
- Familiarity with digital charting and clinical documentation tools
