Hospital Care Coordinator

Position Overview Under the general supervision of the Patient Care Department Manager, the Hospital Liaison – Care Coordinator cultivates strong relationships within multiple medical facilities and is responsible for providing prompt service, educating patients and families on equipment use, coordinating medical documentation, and actively promoting the company’s durable medical equipment, respiratory therapies and infusion services. The role also includes sales-related responsibilities such as identifying referral opportunities, strengthening relationships with healthcare providers, and increasing the company’s presence within assigned hospitals. Responsibilities Serve as the primary point of contact between the company and hospital personnel, ensuring seamless coordination of delivery of equipment to hospital rooms and patient homes. Develop and maintain strong relationships with physicians, case managers, discharge planners, and other health‑care professionals to generate new business opportunities and drive referrals. Promote and educate hospital staff on durable medical equipment, respiratory therapies, and infusion services to increase awareness and referral activity. Conduct regular facility visits to ensure visibility and accessibility for health‑care professionals and patients. Provide timely consultation to patients and families regarding equipment delivery, potential co‑pays and usage instructions. Request and ensure the accuracy of all necessary medical documentation for equipment orders. Monitor and track referrals, ensuring the company is the preferred vendor for partnered health‑care systems. Identify opportunities to expand service utilization and sales within assigned hospital accounts. Conduct or participate in presentations, in‑services and educational seminars for hospital staff and case managers to reinforce the company’s value proposition. Maintain accurate records of interactions with hospital personnel, patient referrals and sales activities, submitting required reports in a timely manner. Ensure compliance with insurance pre‑authorization requirements and coordinate with managed care and primary medical groups as needed. Maintain adequate inventory levels in assigned hospital storage areas to ensure prompt equipment availability and reconcile movement of equipment. Collaborate with customer care specialists and dispatch teams to optimize equipment delivery and customer satisfaction. Represent the company professionally at all times and adhere to all HIPAA, company and industry‑related regulations. Attend all department meetings and training sessions as scheduled. Perform all other duties as assigned by department leadership. Physical Demands Walk frequently across large healthcare facilities. Lift and carry equipment and miscellaneous items up to 75 lbs. on a daily basis. Work Environment Fast‑paced healthcare facilities with an emphasis on prompt deliveries and service coordination. High‑level communication and collaboration with multiple health‑care professionals and vendors. Manage multiple discharges, work under tight deadlines and ensure efficient information transfer between the company and hospital staff. Experience Requirements Experience in medical supply, home medical equipment or related health‑care industries. Strong knowledge of medical terminology and diagnosis codes. Proven experience in customer service and relationship‑building within a health‑care setting. Sales experience in a health‑care or medical‑related field (preferred). Ability to multitask, prioritize and meet deadlines efficiently. Strong organizational skills with attention to detail.
