Summary

The Physical Therapist collaborates with residents and healthcare providers to deliver physical therapy services aimed at enhancing residents' functional capabilities and independence in daily living activities.

Essential Duties and Responsibilities

• Promotes the mission and values of healthcare services.
• Implements evaluative and therapeutic procedures as directed by a physician or authorized healthcare provider while adhering to safety protocols.
• Conducts evaluations according to department protocols and develops a therapeutic plan that includes long- and short-term functional goals, frequency and duration of services, and selected therapeutic modalities.
• Engages residents and primary caregivers in discussions regarding the therapeutic plan, incorporating their feedback into therapy goals.
• Consults with referring physicians and the healthcare team about the care plan and discharge requirements, and attends relevant team meetings.
• Monitors treatment outcomes, adjusting the treatment plan as needed to meet established functional goals.
• Documents all interactions with residents in the Electronic Medical Record, ensuring clarity and accessibility for the care team.
• Recommends and facilitates the acquisition of assistive devices for residents, providing training on their use.
• Educates residents and caregivers, documenting the effectiveness of instruction provided.
• Refers residents to Physical Therapist Assistants as necessary, ensuring coordinated care among therapy professionals.
• Complies with Medicare and other reimbursement regulations, meticulously documenting resident care in accordance with these standards.
• Provides in-service education for nursing and other departments while maintaining clinical knowledge and skills through ongoing learning.
• Contributes to the development of new programs and services within the rehabilitation department.
• Stays informed about professional issues by participating in relevant organizations and reviewing pertinent literature, sharing insights with staff as needed.
• Supervises assistant-level staff according to their training and adheres to state mandates and professional standards, addressing performance issues with management.
• Mentors new staff during orientation, ensuring all necessary competencies and checklists are completed.
• Maintains cleanliness and organization in office and clinical areas and assists in routine equipment cleaning while adhering to infection control procedures.
• Keeps licenses, certifications, and competencies up to date, completing compliance courses annually.
• Performs other related duties as required.

Education and Experience

• Bachelor's degree required; Master's or Doctorate preferred.
• Active Physical Therapist license in the applicable state.
• Basic Life Support (BLS) certification preferred.
• Prior experience in long-term care or skilled nursing preferred.
• An equivalent combination of education and experience may be considered.

Working Conditions/Physical Demands

• Must possess the physical and mental abilities to perform various tasks, including sitting, standing, walking, bending, stooping, twisting, and reaching.
• Ability to lift up to 50 pounds; a two-person lift is required for weights above 50 pounds.
• Regular lifting and carrying up to 20 pounds may be necessary. Some stress may occur due to responsibilities for ensuring quality care.
