About the Role:
A Passionate Helping Hand Home Health Care LLC is looking for a dedicated and compassionate Physical Therapist to join our growing team in Indianapolis, IN. Responsibilities:

• Conduct comprehensive in-home patient evaluations to assess functional abilities and develop individualized treatment plans
• Provide hands-on physical therapy treatments including therapeutic exercises, manual therapy, and mobility training
• Monitor and document patient progress, adjusting care plans as needed to meet evolving goals
• Educate patients and family members on home exercise programs, fall prevention, and safe mobility techniques
• Collaborate with interdisciplinary care teams including physicians, nurses, and occupational therapists
• Maintain accurate and timely clinical documentation in compliance with Medicare and state regulations
• Coordinate discharge planning and recommend appropriate follow-up care or community resources

Requirements:

• Active Physical Therapist (PT) license in the state of Indiana
• Graduate degree (DPT, MPT, or equivalent) from an accredited physical therapy program
• Home health or outpatient clinical experience preferred
• Strong knowledge of Medicare home health guidelines and documentation standards
• Valid driver's license and reliable transportation for home visits throughout Indianapolis
• Excellent interpersonal and communication skills with a patient-first mindset
• Ability to work independently and manage a flexible caseload
