Case Manager: Homeless Health & Mental Health Advocate

A community health organization in Austin is looking for a Case Manager who will provide mental health and substance use interventions to patients, particularly those experiencing homelessness. Responsibilities include assessing patient needs, developing treatment plans, initiating referrals, and advocating for clients within the community. Candidates should hold a Bachelor’s degree in social work and have at least 1 year of relevant experience. Bilingual skills in Spanish are preferred.
