Remote Clinical Optimization Pharmacist - Geriatrics

GalenusCare seeks a Clinical Optimization Pharmacist to join their PACE/Hospice interdisciplinary care team in Mt. Laurel, NJ. This full-time position involves providing comprehensive medication management and clinical support to patients and caregivers. The role requires a PharmD and at least three years of clinical practice in geriatrics or chronic care.
