Outpatient Occupational Therapist or Physical Therapist | Lymphedema

Position Overview: Responsible for evaluating, developing, and implementing individualized physical therapy treatment plans to promote patient health and recovery. Collaborates with interdisciplinary teams, communicates effectively with patients and families, and maintains thorough documentation of assessments and progress. Key Responsibilities: Evaluate patients using various data sources. Develop and modify treatment programs based on ongoing assessments. Deliver direct therapy services tailored to individual patient needs. Coordinate with medical, nursing, and rehabilitation teams. Maintain accurate documentation and progress reports. Foster effective communication with patients, families, and team members. Participate in professional development activities. Qualifications & Skills: Active license as a Physical Therapist in the state of practice. Preferred 1-3 years of experience in physical therapy. Strong communication, assessment, and coordination skills. Ability to work effectively in a team and adhere to safety standards.
