As a Home Health Speech-Language Pathologist, you'll help patients overcome challenges related to speech, language, cognition, communication, and swallowing while improving their independence and quality of life in the comfort of their own homes. Every visit is an opportunity to restore confidence, strengthen communication, and help patients reconnect with the people and moments that matter most. What You'll Do Complete speech, language, cognitive, and swallowing evaluations Develop individualized treatment plans based on patient goals and clinical needs Provide skilled speech therapy services in patients' homes Educate patients and caregivers on communication techniques and swallowing strategies Monitor patient progress and adjust treatment plans as neededCollaborate with physicians and interdisciplinary team members Complete timely and accurate clinical documentation Qualifications Active Virginia Speech-Language Pathologist License Required Certificate of Clinical Competence (CCC-SLP) Preferred Valid Driver's License and Reliable Transportation Strong Clinical Assessment and Documentation Skills Excellent Communication and Interpersonal Skills Home Health Experience Preferred but Not Required Experience with OASIS is a plus Why Human Touch Home Health?
