Job Description
Job Opportunity Position: RN Hospice We are seeking a dedicated and compassionate RN to join our hospice care team. The ideal candidate will coordinate and direct the provision of home nursing care in accordance with physician orders and agency policies. Key Responsibilities:
- Provide high-quality nursing care in patients' homes, ensuring compliance with agency policies and procedures.
- Coordinate with physicians and healthcare professionals to develop and implement care plans.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits for various patient needs including Start of Care, Wound Care, and Follow-up Visits.
- Complete all required documentation within 24 hours of patient visits.
- Participate in periodic office meetings.
- Home visits required, with a willingness to travel up to 75 miles from the home clinic.
- Reliable transportation, current driver's license, and agency-required liability insurance are mandatory.
- Mileage reimbursed at the IRS standard rate; travel compensation is provided from patient home to patient home.
- Day shift: Monday through Friday, from 8:00 AM to 5:00 PM.
- Weekend and on-call requirements may vary based on location and profession.
- Minimum of 1 year of experience in Adult Home Health or Hospice care.
- Recent experience in the specialty area is required.
- Must possess a current RN license.
- Proficiency in HomeCare HomeBase and OASIS is required; familiarity within the last 6 months is preferred.
- CPR and BLS certifications are required.
- This position covers Kent and Sussex County.
- The daily patient load is approximately 5-10 patients, focusing on quality care and documentation.
