Medford, Oregon Work Model: Onsite (Day Shift) Position Type: Full-Time The Physical Therapist provides skilled therapy services to patients with a wide range of diagnoses and functional limitations. This role focuses on patient evaluation, treatment planning, and rehabilitation to maximize independence and improve quality of life in a patient-centered care environment.

Key Responsibilities

• Perform patient evaluations and develop individualized treatment plans
• Provide physical therapy interventions to improve mobility and function
• Deliver care aligned with patient-specific needs and functional goals
• Collaborate with physicians and interdisciplinary teams
• Educate patients and families on rehabilitation and recovery plans
• Monitor patient progress and adjust treatment plans accordingly
• Maintain accurate clinical documentation and records
• Provide professional consultation and recommendations
• Ensure compliance with clinical standards and safety protocols

Must-Have Requirements

• Degree from an accredited Physical Therapy program: Bachelor's OR Master's OR Doctor of Physical Therapy (DPT)
• Active Oregon Physical Therapist License (or ability to obtain upon hire)
• BLS Certification (American Heart Association – upon hire)
• Strong clinical skills in evaluation and treatment planning
