Senior Hospice RN Case Manager Lead Care Team

St. Croix Hospice is seeking an experienced Registered Nurse Case Manager to lead the patient’s hospice care team and craft individualized plans of care in collaboration with the Hospice Medical Director and family.

You will provide direct patient care, administer medications, and coordinate with physicians and allied staff to ensure comfort and quality of life. The role requires an active RN license and strong communication skills, with on-call and weekend duties as part of the schedule.
