RN LTC Non UnionJob Summary and ResponsibilitiesAs a RN at CHI LakeWood Health, you will implement the nursing process across the continuum of care by providing and coordinating patient care, education, and emotional support to patients and their family members.Every day you will work with our patients to evaluate their needs and provide physical or psychosocial care. Your leadership skills will allow you to handle conflicts resulting in a positive outcome.Responsible for the daily care of the patient including assessments, treatments, medications, education and discharge planning. Develops, implements and modifies the patient care plan as indicated by patient's conditions and responses. Uses multidisciplinary team strategies to resolve patient problemsAdminister medications using bar code scanningRespectful and effective communication with all patient populations, physicians, and across all areas of continuum of careMaintains safe and clean working environment by complying with Safe Patient Handling techniquesMaintains continuity among nursing teams by accurate hand off and documentation in the EMREnsures operation of equipment by completing competency checklists; sequestering and completing appropriate TEAM requests for malfunctioning equipment.
