Big Bend Hospice (BBH) is your “Hometown Hospice,” providing compassionate end-of-life care for over 40 years to the Big Bend region. Guided by our values of integrity, respect, and excellence, we are seeking a dedicated and experienced RN Case Manager II to join our Clinical Administration team to cover out new western Service areas in Central Time.The ideal candidate for the RN Case Manager II role is a compassionate and highly skilled clinician who excels at coordinating care across settings while supporting patients and families through the hospice journey. This individual serves as both a clinical expert and care coordinator, ensuring seamless transitions from admission through ongoing care, while promoting quality, compliance, and an exceptional patient experience.Position Duties and Responsibilities:The RN Case Manager II’s responsibilities will include, but are not limited to the following:Evaluates patients and families for hospice admission using approved criteria and proceduresConducts pre-assessment and admission visits in home, hospital, and facility settingsCoordinates with referring physicians, intake teams, and community partners to facilitate timely admissionsProvides education to patients, families, and referral sources regarding hospice philosophy, eligibility, and servicesEnsures all admission documentation is accurate, complete, and submitted within required timelinesDevelops, coordinates, and implements individualized interdisciplinary group (IDG) plans of careProvides skilled nursing care, symptom management, and patient/family education aligned with hospice best practicesMaintains consistent communication with physicians, IDG members, and community partners to ensure continuity of careSupervises and provides guidance to Licensed Practical Nurses (LPNs) and Hospice Aides (HAs), including initiating plans of careParticipates in interdisciplinary team meetings, quality initiatives, and continuous improvement effortsDemonstrates competency in pain and symptom management, end-of-life care, and holistic patient supportMaintains accurate, timely, and compliant documentation within the electronic medical record (EMR)Ensures compliance with Medicare, Medicaid, and all applicable regulatory requirementsAdheres to all BBH, state, and federal policies and proceduresServes as a clinical resource and mentor to peers, new hires, and studentsCollaborates with Admissions, Education, and Community Relations teams to support smooth care transitionsMaintains a balanced caseload aligned with organizational standardsParticipates in weekend, evening, and holiday rotations as assignedDemonstrates accountability, sound clinical judgment, and effective prioritizationUtilizes resources and supplies in a fiscally responsible mannerRepresents Big Bend Hospice with professionalism and compassionPerforms other duties as assignedWork Schedule:Monday-Friday: 8:00 am- 5:00 pm.Travel Requirement: You must have a reliable vehicle and be willing to be dispatched throughout our service areas.This position will cover the following counties:GadsdenLibertyCalhounJacksonGulfBayHolmesWashingtonother counties as assignedEducation & Experience:Minimum of one (1) year of inpatient nursing experience requiredHospice, home health, or long-term care experience preferredActive Florida Registered Nurse (RN) license requiredSkills & Abilities:Excellent verbal and written communication skillsStrong interpersonal, conflict resolution, and relationship-building skillsExceptional organizational skills and attention to detailStrong clinical judgment, analytical thinking, and problem-solving abilityAbility to prioritize, delegate, and manage multiple responsibilitiesDemonstrates integrity, professionalism, and confidentialityProficient in Microsoft Office Suite and ability to learn internal systemsAbility to manage the physical and emotional demands of caring for terminally ill patients and familiesMust communicate effectively in EnglishRequirements:Current Florida Driver’s LicenseValid auto insurance and reliable transportationCommitment to BBH’s core values: integrity, compassion, and accountabilityTotal Rewards:Big Bend Hospice offers a robust Total Rewards package to include Competitive Salary, Robust Employee Benefits, Recognition and Public Service Loan Forgiveness (eligible for most roles).Apply today and help us grow our mission to inspire hope by positively impacting the way our community experiences serious illness or grieve - one family at a time!Big Bend Hospice is an equal opportunity employer committed to diversity and inclusion. #J-18808-Ljbffr Big Bend Hospice
