Care Career is seeking a travel nurse RN Case Management for a travel nursing job in Margate, Florida.Job Description & Requirements

Specialty: Case Management

Employment Type: Travel

Roles & Responsibilities

Assess patients' medical, psychosocial, and discharge planning needs.
Develop and coordinate individualized care plans.
Collaborate with physicians, nurses, social workers, and other healthcare professionals.
Facilitate discharge planning and transitions of care.
Coordinate post-acute services such as home health, rehab, SNF, or DME.
Monitor patient progress and adjust care plans as needed.
Ensure appropriate resource utilization and length of stay management.
Advocate for patients and families throughout hospitalization.
Maintain accurate documentation in Meditech and other case management systems.
Support quality outcomes, patient satisfaction, and cost-effective care.

Required Qualifications

Active RN License.
BLS Certification.
Acute Care Hospital Case Management experience.
Strong discharge planning and care coordination skills.
Experience working in a fast-paced hospital environment.
Excellent communication, organizational, and multitasking abilities.
Ability to work independently and as part of a multidisciplinary team.
Adaptable, reliable, and positive attitude.
