Responsibilities:

• Conduct initial evaluations to assess patients' needs and develop individualized treatment plans
• Develop and implement appropriate treatment plans based on patient needs and goals
• Monitor patient progress and adjust treatment plans as necessary
• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
• Maintain accurate and timely documentation of patient progress
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care
• Provide home health and/or outpatient services as needed, including supervisory visits, to ensure optimal outcomes

Qualifications:

• A degree in Occupational Therapy from an accredited program
• Must be licensed or registered by the applicable State without restrictions.
• One year of home health experience required.
• Must have knowledge of Medicare and Medicaid guidelines
• Must have a valid driver’s license and reliable transportation.
• Must have a current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.

Background Screening Requirement: Employment in Florida is contingent upon completion of a Level 2 background screening in accordance with Florida AHCA requirements. This position is available on a PRN basis only. Hours are not guaranteed and will vary depending on business needs, staff coverage requirements, and workload demands.
