Travel Medical Social Worker - Full-Time

Medical Social Worker Role & Responsibilities Provide initial assessments within 24 hours and coordinate discharge plans for patients. Manage a caseload of 20-23 patients across various diagnoses and age groups. Collaborate with a multidisciplinary medical team to ensure comprehensive care. Assist patients in navigating community resources and post-acute care services. Document care activities thoroughly, participate in care conferences, and support interdisciplinary activities. Float between facilities as needed and work during holidays and every other weekend to ensure coverage. Demonstrate strong crisis intervention skills and flexibility in scheduling. Qualifications & Skills Minimum one year of acute care experience. California state license required; pending license accepted but must be licensed upon start. Certifications: BLS, LCSW, MSW. Experience in documentation, care coordination, and discharge planning. Excellent communication, crisis intervention, and community resource knowledge.
