As our Memory Support Director , you'll be the heart and leader of a vibrant memory care neighborhood-creating meaningful moments, inspiring your team, and directly impacting the lives of residents.

In this rewarding role, you'll:

Bring the Portraits Program to Life
Lead our Montessori-inspired memory care program, creating personalized experiences that support each resident's physical, emotional, social, spiritual, and cognitive well-being.

Get to Know Every Resident's Story
Conduct resident evaluations before move-in and throughout their journey, ensuring care plans and service agreements evolve to meet their unique needs.

Coach and Inspire an Amazing Care Team
Train, mentor, be part of a team, and develop caregivers while fostering a culture of compassion, excellence, teamwork, and continuous dementia education.

Champion Outstanding Care
Monitor quality, conduct care observations, review documentation and response times, and implement improvements that help ensure exceptional resident experiences every day.

Collaboration and Teamwork

Serve as a key collaborator across the community, working closely with the Executive Director, healthcare professionals, dining services, and departmental leaders to foster a coordinated and resident-focused environment.

Build Meaningful Connections
Develop trusted relationships with residents, families, team members, healthcare partners, hospice providers, home health agencies, and regulatory professionals.

Make a Difference Every Day
Create an environment where residents thrive, families feel supported, and team members are empowered to deliver compassionate, person-centered care.

Requirements

What We're Looking For:
  • 2+ years of experience and knowledge of health-related practices related to the care of persons living with dementia or cognitive impairment.
  • 1+ years of management experience including hiring staff, coaching, performance management, daily operations supervision, discipline, and counseling (preferred).
  • An associate's degree in nursing, social work, or another health-related field (preferred).
  • A Montessori Certification, or the willingness to obtain it.
  • A current CPR/BLS certification.
  • Valid driver's license and acceptable driving record.
  • The ability to assist residents with limited mobility, lift/push/pull up to 50lbs.
  • Excellent verbal and written communication skills, with the ability to effectively collaborate with diverse groups, navigate challenging situations, and make sound decisions.
  • Strong technical skills (e.g., Word, Excel, PowerPoint) and the ability to maintain confidential and privileged information (including HIPAA, residents, and associates).
