$54,745 - $71,156 based on experience

401K Join the management team at our DeSoto location and see how you can make a meaningful difference in the lives of individuals with disabilities. Responsibilities: Develop and implement Individual Service Plans, based on assessments and observations. Responsible for 24 hour a day Group Home with rotating on-call and scheduling. Supervise, plan, assign, and evaluate or monitor programming and the work of staff responsible for the daily care of individuals. Work closely with therapists, service coordinator, administrations, and parents/guardians to maintain communication and provide feedback, standardized procedures, expedite support plan implementation and work-flow, and to improve the efficiency of all care/support providers. Performs various assessments; chart reviews; documentation, mealtime and environmental monitoring.
