Key Highlights Weekly pay: $1,999-$2,115 per week Guaranteed hours: 35 hours per week Contract duration: 13 weeks Shift: 8.5-hour day shifts, 5 days per week (08:00–16:30) Start date: 05/04/2026 Setting: Skilled Nursing Facility (travel assignment) Responsibilities Conduct PT evaluations and develop individualized treatment plans for residents Implement therapeutic interventions including gait training, balance, mobility, strength, and functional activities Monitor progress, document outcomes, and adjust plans of care in the electronic health record per policy Collaborate with an interprofessional team (nursing, therapy colleagues, and ancillary staff) to optimize resident outcomes Educate residents and families on exercises, safety, and home programs to support continued improvement Maintain patient safety, adhere to infection control practices, and ensure compliance with standardized protocols Requirements Active Physical Therapist license in the United States; NH licensure or eligibility to obtain NH licensure Graduation from an accredited physical therapy program Ability to perform duties in a skilled nursing facility setting and interact with geriatric populations Flexibility to work a 5x8 day shift and participate in a 13-week travel assignment Strong communication, documentation, and collaboration skills Benefits and Support Dedicated travel assignment support through Healthforce Healthcare LLC Assignment-specific scheduling and supervision as applicable to a travel PT role Onboarding and credentialing assistance to facilitate temporary licensure and clearance Please note: Pay rates and guaranteed hours are estimates and may vary.
