Summary Description Of Work This position connects those experiencing homelessness with housing in the form of temporary financial assistance and supportive services in Douglas, Sarpy, and Pottawattamie Counties. This Case Manager will work to assess, verify eligibility, assist clients with locating and securing housing, and provide case management/supportive service to clients in the program using the Housing First model. Intensive case management responsibilities begin at this level. Educational requirements are specific. Decision making, problem solving, and highly developed interpersonal skills are critical. This position requires high level of social work skills, practice, and knowledge. Compensation: between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience) Work Schedule: 37.5 hours per week Minimum Qualifications Bachelor's Degree, and two years direct care experience in Human Services; or equivalent six years of case management experience in Human Services Valid Drivers License and acceptable driving record Essential Duties And Responsibilities Essential functions of this job are performed on company physical work site. Participate in outreach to homeless individuals to engage them in services and screen for program eligibility Conduct intakes and assessments to accept, deny, and refer applicants Verify and document the eligibility of applicants and maintain comprehensive service records for all approved or denied clients Assist individuals in locating, securing and retaining suitable housing Help clients identify strengths, needs and goals, including meeting obligations of tenancy to maintain their home Develop an individualized housing stability plan in cooperation with the client and monitor and evaluate program participant progress Assist clients in increasing financial security by accessing, maintaining or improving employment and/or benefits Assist program participants with critical skills related to household budgeting, money management, accessing a free personal credit report, and resolving personal credit issues Provide problem solving and crisis intervention services to empower clients to develop self-sufficiency Provide case management services related to housing needs of program participants in helping them obtain housing stability such as teaching/coaching clients to develop communication and self- advocacy skills Ensure safety and security measures for in-home services are followed Ensure required information is collected, entered into prescribed formats, and reported in a timely manner as outlined by Federal funding and internal reporting guidelines Develop and maintain positive working relationships with landlords, clients, referral sources, and others encountered in the course of work Demonstrate knowledge of current community and Heartland Family Service programs and resources to effectively meet client needs Evaluate efficiency and effectiveness of community service providers on an ongoing basis to ensure clients are receiving quality care Work independently with little supervision and as part of a team Utilize trauma-informed care and culturally competent principles in providing services to clients Work is completed in client homes, homeless shelters, office space, and other non-traditional settings Travel is required; use personal vehicle if agency vehicle is not available Establish regularity and dependability in scheduling all appointments and accuracy in reports and statistics Display a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency Abide by all specific program and Agency procedures, policies, and requirements Cooperate and collaborate with all programs staff, volunteers, landlords and referral sources Be dependable and punctual regarding scheduling and attendance Evaluate HFS program services and make recommendations Develop personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media Create, maintain, and share as appropriate a dynamic self-care plan Strive to make connections between the agency and the larger community whenever possible in order to contribute to the agency's ongoing fundraising and friend-raising efforts Perform other program related duties as assigned #J-18808-Ljbffr Heartland Family Service
