GENERAL SUMMARY AND SCOPE The PGY1 Resident Clinical Staff Pharmacist assists the Director in providing specialized pharmaceutical services to include advising physicians on matters pertaining to drug usage and control within assigned hospital locations; and participates in the development and delivery of pharmaceutical and clinical services and continuing education programs for Pharmacy, Patient Care Services, Medical staff and students/residents.

ESSENTIAL TASKS, DUTIES AND RESPONSIBILITIES
- Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including, but not limited to, advising physicians on issues concerning drug therapy, the inherent toxicity of drugs and their side effects, as well as assisting in the prescription of appropriate medications and doses. Coordinate the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with physicians' prescriptions; ensure compliance with federal, state and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records.
- Mentor Pharmacy Practice Students and Medical Students; activities should include both practical and didactic experiences.
- Maintain liaison relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls.
- Conduct and evaluate medication histories, assess compliance and suggest modifications so as to achieve desired outcomes; instruct patients in the proper use of prescribed drugs and make Patient Care Rounds with physicians to evaluate patient progress.
- Individualize medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of Pharmaceutical Care to patients within areas of direct responsibility and assisting others in the department with the same.
- Participating regularly in the pharmacy outpatient clinics is also an integral part of the clinical/staff pharmacist's responsibilities. Serving on the hospital's numerous patient safety and quality committees will also be expected.
- Conducting and participating in research including, but not limited to, performing as the primary investigator or co-investigator for research programs which will impact the delivery of quality care or examine the pharmacoeconomic impact of providing care to the patients in the Firelands Health System.
- The above statements describe the general nature and level of work being performed by individuals assigned to this classification. This is not intended to be an exhaustive list of all responsibilities and duties required of personnel so classified.
- Clinical Staff Pharmacists supervise and direct supportive personnel including non-clinical staff pharmacists.
- The PGY1 Resident Clinical Staff Pharmacist will be responsible for performing any and all duties required by the residency program director with regard to accreditation of the pharmacy residency program at Firelands and ultimately, graduation from the residency program.
- Interprets drug orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete pharmacy patient medication profiles.
- Compounds and dispenses pharmaceuticals including sterile products, chemotherapy and parenteral nutrition products accurately.
- Issues controlled substances to patient care areas and maintains records as required by law.
- Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
- Assists with pharmacokinetics consult service and drug dosing per hospital protocol.
- Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes as needed.
- Reads, extracts and interprets information in patient charts accurately.
- Detects and reports suspected adverse drug reactions accurately and in a timely manner.
- Sustains the hospital drug formulary, minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.
- Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
- Provides accurate, adequate, and timely drug information to other members of the hospital's professional staff.
- Provides drug education to patients and their families.
- Documents all clinical activities and interventions accurately and completely.
- Participates in the quality improvement and medication use review activities of the department. Collects data, conducts monitors and inspections and maintains logs, records and other documentation as assigned.
- Participates in the development and presentation of orientation, education, and training programs for the medical staff, nursing service, and other hospital staff.
- Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel.
- Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner.
- Completes all assigned medication storage area inspections in a timely manner. Identifies and replaces outdated and unusable drugs.
- Answers the telephone, identifying self and department. Directs calls to appropriate personnel. Answers requests at the window.
- Keeps pharmacy areas and equipment clean, neat, and well-organized.
- Oversees operation of the pharmacy in the absence of the Director as necessary.
- Maintains current pharmacist license. Ensures applicable CE records and licensure are maintained in department files.
- Attends Pharmacy staff meetings.
- Attends orientation, education, and training programs. Reviews literature and other materials pertinent to the practice of pharmacy.
- Completes all competency/skills assessment requirements.
- Develops and implements initiatives to ensure safe and cost-effective management of pharmaceuticals in collaboration with the health care team.
- Develops, implements, and actively participates in all residency requirements, clinical research projects and other scholarly activities as appropriate.
- Responsible for special projects pertaining to specific areas of interest and/or expertise; for conducting in-depth and highly specific research reviews of drug-related literature and ability to make presentations on rational drug therapy.
- Responsible for some inventory control/purchasing activity as required to maintain departmental inventory at appropriate levels.
- Actively participates in the accomplishment and ensures the implementation of the pharmacy service's scope of practice.
- Types labels for medications as well as IV solutions when needed.
- Answers telephone, receiving and relaying messages as needed.
- Participates in department performance improvement activities as assigned and attends required training.
- Performs other duties, as assigned.
- Graduate of an ACPE accredited School of Pharmacy with a BS Pharmacy degree or Pharm.D. Currently licensed or seeking licensure examination to practice pharmacy in the state of Ohio.
- Ability to work independently with minimal direct supervision. Ability to work cooperatively with pharmacy staff, hospital staff, and physicians.
- Ability to handle frequent interruptions and adapt to changes in workload and work schedule.
- Ability to set priorities, make critical decisions and respond quickly to emergency requests.
- Fosters a team environment by providing orientation and training to new team members.
- Ability to be mobile.
- Ability to communicate effectively (verbal and written).
- Punctual attendance at assigned work location is required.
- Ability to work in a safe and efficient manner and maintain an accident free work place, including ability to demonstrate a working knowledge of Medical Center emergency codes.
- Ability to comply with OSHA regulations and CDC standard and transmission-based precaution recommendations and to utilize proper personal protective equipment. Ability to comply with provisions of applicable S.D.S. forms.
- Successful completion of a ninety (90) day probationary period.
