Hospice RN Case Manager - Home-Based Care Leader

Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and provide clinical, palliative and supportive care to keep patients in their home environment when possible. Requirements include an active RN license in Florida, 1+ year nursing experience (hospice/hospital preferred), Alzheimer’s training for PACE locations, EMR/EHR experience, and a valid driver’s license with reliable transportation. #J-18808-Ljbffr Chapters Health System
