Key Responsibilities

• Provide occupational therapy services to patients across an assigned county territory in the Vacaville, Fairfield, Rio Vista, Suisun City, Dixon, Vallejo, Benicia, Napa, American Canyon, and Yountville areas
• Complete OASIS assessments including recertifications, resumptions of care, start of care evaluations, and discharge visits
• Conduct routine visits, re-evaluations, and follow-up visits in accordance with patient care plans
• Maintain expected weekly productivity standards, which may include occasional travel outside the primary service area or weekend coverage as needed
• Communicate effectively with patients and physicians, including documentation of all visit-related activities
• Be prepared to see patients with infectious conditions as required by assignment needs

Required Qualifications

• Current California Occupational Therapist (OT) License
• BLS Certification
• Valid Driver's License, current auto insurance, and current vehicle registration
• Up-to-date immunizations and health screenings: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), Flu (annual)
• Ability to pass a drug screen and physical
• New graduates are welcome to apply

Preferred Experience / Skills

• Prior home health experience is preferred but not required
• Familiarity with OASIS documentation and home health visit workflows
• Comfort with a field-based, self-directed work environment across a multi-city territory
