- EMR Experience (Epic or Curo) Job Description The LVN Case Manager will support patients through the Pathways program, providing outreach, enrollment, and ongoing care coordination. This role helps bridge gaps in care by connecting patients with specialists, coordinating referrals, and supporting patients throughout their care journey.\n \nKey Responsibilities:\nConduct patient outreach and enroll eligible patients into the Pathways program\nComplete approximately 25 meaningful patient conversations per day\nSpend roughly 50–60% of the day on the phone engaging with patients\nCoordinate referrals and connect patients with appropriate specialists and resources\nIdentify and help close gaps in care through ongoing patient follow-up and coordination\nCollaborate with the broader care team to support continuity of care\nSchedule / Location:\n3 days per week: Highland\n1 day per week: Ontario\n1 day remote
