Outpatient Physical Therapist

This is a 13-week contract position.

Shift: 5x8.0

The Outpatient Physical Therapist is responsible for evaluating all assigned patients and developing, implementing, and modifying treatment plans according to the patient's needs, goals, and physician direction. The therapist will delegate and direct therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities. Educating patients, families, students, coworkers, and themselves is a vital aspect of this role.

Complete comprehensive evaluations of assigned patients using clinical reasoning.

Develop and implement treatment plans focused on progressing patients toward discharge.

Maintain medical records, ensuring documentation is clear, concise, accurate, and timely.

Provide care in compliance with signed, valid physician orders and within approved insurance guidelines.

Participate in the student program as assigned, fostering a positive learning environment.

Create and pursue an individualized professional development plan with clinical management.

Stay current with continuing education and maintain licensure in good standing.

Adhere to ethical standards and the discipline-specific professional code of conduct.

Engage in performance improvement initiatives to enhance the quality of services provided.

Degree in Physical Therapy from an accredited school.

Minimum of 1 year of outpatient clinical experience.

Current CPR certification.

Active and applicable state licensure.

Previous assignments with this agency are highly desired.
