Essential Duties And Responsibilities Deliver direct patient care as assigned, using established procedures in the patient’s home environment. Travel to patients’ homes and manage their care plans according to physicians’ instructions. Maintain accurate, detailed reports and records on medical information, vital signs, or changes in patient’s conditions. Monitor patients' medication usage and results. Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes. Evaluate patients' behavior to formulate diagnoses or assess treatments. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans. Provide personal assistance, medical attention, emotional support, or other personal care to patients. Coordinate with other home health professionals caring for the patient, and communicate the patient’s needs to the family providing support. Train other nurses, coordinate with other home health professionals caring for the patient, and communicate the patient’s needs to the family providing support. Assure the continuity of care through the delivery of quality patient/client care. Provide effective communication to patient/client/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes. Complete appropriate documentation in a timely manner. Monitor assigned cases to ensure compliance with physician orders and requirements of third party payor. Demonstrate commitment, professional growth, and competency. Other duties as assigned.
